DRMC Patient Safety - ANONYMOUS REPORT
Pennsylvania's Patient Safety Authority

The anonymous reporting form is used by healthcare workers to report a serious event to the Commonwealth of Pennsylvania's Patient Safety Authority. A serious event is an event, occurrence, or situation involving the clinical care of a patient in a medical facility that:
EITHER
A. Resulted in the patient's death, or
B. Resulted in an unanticipated injury to the patient that required additional healthcare services AND compromised patient safety.

Before reporting a serious event to the Patient Safety Authority, make sure you or someone else has filed a report according to your facility's Patient Safety Plan.
