• Discuss your concern (s) with the staff present; and/or
• Ask for the Department Manager or Supervisor; and/or
• Call Risk Management Department (375-3528); and/or
• Also send concern to Risk Management Department 100 Hospital Ave. DuBois, Pa 15801.

Should you feel your concerns are not adequately addressed or prefer not to discuss with hospital personnel,
